I agree with 1,3,4,5&8.. not so many persons are keen on producing quality and some customers as well doesn’t mind less quality so far the quantity is high and sopme team needs adequate supervision to produce quality product/services… I believe quality criteria should be agreed upon from the inception of the project which should stem from the purpose the product is to serve and how it satisfy the customer’s needs.
